Care Management

If you or a loved one is living with a chronic health condition such as diabetes, high blood pressure, or chronic obstructive pulmonary disease (COPD), a nurse can help you manage your symptoms. For details, contact a UnitedHealthcare Health Advocate at 1-844-298-8934 or visit the myuhc.com® website.

Additional support programs are available specifically for maternity, cancer, care decision, and bariatric surgery. Learn more by clicking on the links below. To enroll, call a UnitedHealthcare Health Advocate at 1-844-298-8934

This program gives expectant mothers help through every stage of pregnancy and delivery. It’s best to enroll within the first 12 weeks of your pregnancy, but you can enroll through week 34.

The program is provided to you at no extra charge as part of your health plan benefits.

Get in touch with an experienced cancer nurse who can help explain the diagnosis and treatment options. This resource is available at no cost to you as part of your health plan benefits.

Cancer nurses are caring, friendly professionals that serve as a reliable guide to help you or a family member:

  • Find health care providers
  • Schedule appointments and have medical records transferred
  • Learn about treatment information
  • Receive overall support

And to help you get the best possible outcomes from your cancer treatment, UnitedHealthcare has identified Centers of Excellence for cancer and can help you identify a quality provider at a Center of Excellence in your area.

Need medical treatment or a recommended procedure but not sure where to go? Decision support experts will work with you to understand your options, find providers, estimate out-of-pocket costs, and more, so you can get the care that’s right for you.

Receive information and support if you are considering bariatric surgery. Experienced nurses will help you understand and manage the conditions associated with obesity before and after surgery.

Bariatric surgery must be completed at a Center of Excellence facility.

If you enroll in medical coverage through DriveTime, SurgeryPlus will help you find a board-certified surgeon for many different surgeries. Save money with negotiated costs that, in many cases, cover your out-of-pocket deductible and coinsurance costs.

To learn more, visit the SurgeryPlus website (access code: DriveTime).
